Being able to access your medical information is  important  to managing  your health. Memorial Hermann is excited to make this process simpler for you and your loved ones with My Memorial Hermann, your new patient portal.

As we transition your existing patient portal, Everyday Well, to My Memorial Hermann, your ability to manage appointments at Memorial Hermann facilities will transition as well.

To view your appointments with Memorial Hermann Medical Group (MHMG), Mischer Neuroscience Associates (MNA), Health Centers For Schools, Neighborhood Health Clinics or other select affiliated Memorial Hermann practices, please download and sign up for My Memorial Hermann.

All other appointment types will transition to My Memorial Hermann in Fall 2024 and you can continue to manage those appointments and access your health information from within Everyday Well. These include appointments with Memorial Hermann Outpatient Imaging, Sports Medicine & Rehabilitation, TIRR, Sleep Labs, Diabetes Education, our Hospital Inpatient Units, the Emergency Centers and Hospital Based Clinics for Oncology, Wound Care, Nephrology, Outpatient Cardiology and some procedural areas.

Note: At Memorial Hermann Health System, we are dedicated to protecting the information of our patients. The proxy process was designed to comply with state and federal privacy laws. The Everyday Well patient portal has certain technological limitations which impact proxy access to medical records for patients ages 13-17. Therefore, proxy access through Everyday Well is not available to parents for this age range.

Parents accessing the My Memorial Hermann and Epic MyChart patient portal are able to see portions of medical record for patients ages 13-17. Parents may contact the Memorial Hermann Release of Information Department directly at (713) 222-CARE (2273) or online at Request Medical Records to request a complete copy of their child’s medical records.
