What is Home Health Physical Therapy (PT)?

With home health physical therapy, a licensed therapist comes to your home to work with you, following a specific treatment plan. Home health PT is a specialized program for patients who need to stay in their homes instead of coming into a facility for therapy. These patients include those who have an acute health concern, or those who do not have the functional mobility to safely leave home. Home health PT is covered by most insurance plans.

Physical therapists help patients return to a healthier, more enjoyable life. Through a combination of hands-on therapy and guided exercises, physical therapists help patients function at their full potential. These skilled, licensed professionals are important to the overall care team and will coordinate with your physician to provide you with the best possible treatment.

Improve Mobility and Functional Status at Home

When you are recovering from an injury or illness, physical therapy can help restore your daily functional abilities and help get you back to the life you enjoy. If you are unable to safely leave your home, the specialty trained physical therapists at Memorial Hermann Home Health can come to you to guide you on your road to recovery. Our goal is to improve your strength and quality of life by increasing motion and mobility in the comfort of your home.

Who Can Benefit from Home Health PT?

Patients who need physical rehabilitation but cannot leave home can benefit from home health PT. Physicians often recommend home health PT for patients who have undergone certain types of surgery or have other illnesses:

  • Post-surgical care for procedures like rotator-cuff repair or total knee replacement
  • Stroke
  • Acute cardiac conditions, including heart attack
  • Physical trauma, including broken bones

What to Expect

At Memorial Hermann Home Health, our physical therapists are trained specialists with extensive experience in determining which treatment options will be most successful, based on medically proven methods and techniques. In your therapy, you can expect to follow a clinically based plan with specific goals.

Your First Visit

The first visit from your therapist will focus on evaluation and planning. Your therapist will conduct a comprehensive exam to assess your health and determine appropriate goals. Next, the therapist will work with you to develop a specialized treatment plan to help you achieve your treatment goals.

As You Make Progress

Most treatment plans also include exercises to do on your own, between visits from the physical therapist. During home visits, your therapist will evaluate the progress you are making and adjust your treatment plan, if necessary. As part of a comprehensive approach, your therapist will stay in communication with your physician, keeping them informed of your progress toward your health goals.
